Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

The area of concern was observed to contain drywall walls, carpeted floors, and drop ceiling tile. Wood cabinetry associated with a sink was also observed in the room.
The carpet was evaluated for moisture using a moisture sensor. No elevated moisture was identified in the carpet.
The drywall walls and cabinetry were evaluated using a moisture meter. No elevated moisture was identified within the walls and cabinetry.
No visible mold was observed within the room or within the plenum.
A malodor was observed within the space. Based on conversations with staff, a leak originating from the Heating, Ventilation, and Air Conditioning (HVAC) system occurred during the spring of 2026. Staff members believed that the odor was connected to the historic leak and carpet.
A stained ceiling tile was still present where the HVAC leak originated.
Custodial staff extracted and cleaned the carpet to determine if odor was caused by debris, dust, and particulates from the HVAC leak and normal use.
Custodial staff and the Office of the Environment did not observe malodors after the carpet was cleaned and extracted.
