Concern
Condensation, as well as water from the roof, has been showing on the ceiling tiles in our student service office. When the tiles were removed this morning (9/24/26), they found black material on all the piping. The tile has been replaced, but the potential mold is still there.
IEQ Investigation Process

Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:

  • interview/questionnaire of concern individual(s)
  • inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
  • inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
  • inspection of exterior
  • inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)
Findings

Date of Assessment: September 28, 2026

The Office of the Environment observed condensation on pipe insulation associated with the heating, ventilation, and air conditioning (HVAC) system in Room 904.

Staining and discoloration were observed on the pipe insulation; however, no obvious indications of mold growth were observed.

Ceiling tiles exhibiting staining and moisture were observed beneath the affected pipe insulation, consistent with condensation.

The HVAC shop was aware of the issue and had scheduled an insulation contractor to replace the affected insulation at a later date.

As a precaution, the remainder of the office was assessed for potential indoor environmental quality (IEQ) concerns.

Carpet flooring was assessed using a moisture sensor, and no elevated moisture levels were detected at the time of the assessment.

Drywall walls were assessed using a moisture meter, and no elevated moisture levels were detected at the time of the assessment.

No visible indications of mold growth or water intrusion were observed within the space beyond the previously identified condensation issue.

Corrective Actions
September 28th, 2026

As a precaution, the Office of the Environment cleaned pipe insulation with a disinfectant.

October 16th, 2026

HVAC pipe insulation was replaced by a board approved contractor.
