Concern
There is a musty/moldy smell coming from room 23.
IEQ Investigation Process

Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:

  • interview/questionnaire of concern individual(s)
  • inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
  • inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
  • inspection of exterior
  • inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)
Findings

Dates of Assessment: September 15, 17, and 18, 2026

September 15, 2026

The room contained a combination of drywall, concrete masonry units (CMU), and modular walls, as well as vinyl floor tiles and ceiling tiles.

Temperature, relative humidity, and carbon dioxide measurements were collected and were within acceptable ranges at the time of assessment.

A unit ventilator was observed within the space and provides cooling for the room. The filters were reported to have been replaced in August. The coils appeared clean, and no visible debris or condensation was observed within the drain pan.

The walls and ceilings were assessed using an infrared thermal camera. No thermal anomalies indicative of water intrusion were observed.

The wood shelving surrounding the unit ventilator was assessed using a moisture meter. Elevated moisture was identified in the shelving on the left-hand side of the unit ventilator.

No visible or olfactory indications of mold growth were observed.

September 17, 2026

Building Maintenance (Carpentry) removed the shelving and identified that the drain line from the unit ventilator was disconnected and leaking onto the carpet beneath the shelving.

The carpet was reported to have been impacted by moisture.

The Office of the Environment was contacted by the Carpentry Shop to conduct an assessment after the wet kickplate and carpet had been removed and prior to installation of the replacement shelving.

September 18, 2026

The Office of the Environment assessed the remaining shelving materials. No visible mold growth was identified on the moisture-impacted materials or on the unit ventilator supply pipes located behind the shelving unit.

Corrective Actions
September 17th, 2026

Building Maintenance (Carpentry) replaced the drain line back into the outlet. 

September 18th, 2026

Building Maintenance (Carpentry) installed a new kickplate and reinstalled shelving unit.
