Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

Temperature, relative humidity, and carbon dioxide measurements were collected within the space. Relative humidity was found to be significantly elevated at 91.2% at the time of assessment.
The walls and ceilings were evaluated using a thermal infrared camera. No thermal anomalies indicative of active water intrusion were observed at the time of the assessment.
No visible indications of mold growth were observed within the assessed space.
A dollhouse located within the space was observed to have significant dust accumulation.
A musty odor was noted within the space at the time of the assessment. The elevated relative humidity may have contributed to or intensified the observed odor. High humidity can also promote odor retention in porous materials and accumulated dust, potentially contributing to persistent musty odors within an enclosed space.
Staff member was informed about significant dust accumulation on doll house and the potential indoor environmental quality concerns related to dust. Staff member was also informed that custodial staff is not responsible for cleaning personal items.
Staff member dusted the doll house in response.
Building Maintenance (HVAC) identified a controls issue within the Building Automation System (BAS) that contributed to the elevated relative humidity.
The controls sequence was corrected.
