The wall in B3 was found to have moisture. The identified solution was to pull the dry wall and runs fans. That did not happen, and has not yet happened. There is a strong mildew/mold smell in the office.
Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

Speaking with school administrators, water intrusion issues were caused by lack of flashing being installed in conjunction with the building envelope renovation.
The general contractor utilized a board approved restoration contractor to complete the removal of impacted materials and reinstallation.
No active water intrusion was present at the time of assessment.
The carpet was assessed for moisture utilizing a moisture sensor. Elevated moisture was observed underneath a recycling bin. Carpet tile was removed and visible signs of moisture and "musty" odor was observed.
Based on conversations with staff, moisture has most likely been trapped underneath a recycling bin since custodial cleaning activities prior to school starting.
No visible or olfactory indications of mold growth were identified.
An infrared camera was utilized to assess drywall, windows, and ceilings. No thermal anomalies indicative of moisture intrusion were observed.
Custodial staff to reclean carpet and place fans on area to facilitate drying and prevent musty odor from returning.
Building Maintenance (Carpentry) to replace carpet tile.
