Concern
So there is a weird leak in one of the pipes and it causes the ceiling tile to saturate and to drip onto the floor, it eats the floor wax and every few weeks they just switch the ceiling tile.

IEQ Investigation Process

Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:

  • interview/questionnaire of concern individual(s)
  • inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
  • inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
  • inspection of exterior
  • inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)
Findings

Date of Assessment: August 28, 2026

The Office of the Environment observed the stained ceiling tile within the hallway outside of S-155. 

The ceiling and adjoining wall were assessed using a thermal camera. A thermal anomaly indicative of water intrusion was observed on the ceiling tile. 

The Office of the Environment assessed materials above the ceiling tile for source of water intrusion. A water line was observed leaking onto the ceiling tile. 

No visible or olfactory indications of mold were observed below or above the ceiling tiles. 

Corrective Actions
September 4th, 2026

Building Maintenance (Plumbing) placed a patch on the line as a temporary measure on August 28, 2026. 

On September 4, 2026, plumbing observed that the patch was still holding.

Building Maintenance (Plumbing) to schedule partial shutdown in order to replace portion of the line.
