Concern
A ceiling tile had mold on it. The tile was replaced, and the moisture is back again. HVAC has been informed, but we are concerned that the original mold may be elsewhere in the ceiling.
IEQ Investigation Process

Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:

  • interview/questionnaire of concern individual(s)
  • inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
  • inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
  • inspection of exterior
  • inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)
Findings

A stained ceiling tile was observed within Room 183. 

A Variable Air Volume (VAV) box associated with the HVAC system was observed above the stained ceiling tile. 

No indications of condensation or leaking were observed in connection with the VAV Box or associated piping. 

No visible or olfactory indications of mold growth were observed above the ceiling tile. 

The stained ceiling tile, VAV Box, and piping, and ductwork were assessed with an infrared camera. No thermal anomalies indicative of moisture intrusion were observed using the infrared camera. 

Based on conversations with custodial staff, the stained ceiling tile in this room and other areas were caused by improper settings causing the VAV boxes to condensate. 

Corrective Actions
August 19th, 2026

The HVAC programming was reconfigured to have VAV Boxes shut off during unoccupied hours.
