Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

Visible mold growth affecting less than one square foot was observed on the drywall ceiling.
The affected area was evaluated using a moisture meter. No elevated moisture levels were detected, indicating that an active moisture source was no longer present.
The Office of the Environment removed the affected drywall to determine whether mold growth on the upper surface of the ceiling extended beyond the initially observed area. The mold growth was confined to the original area of less than one square foot.
The ceiling was evaluated using a thermal infrared camera. No thermal anomalies indicative of active moisture intrusion were identified.
Building Maintenance (Roofing) investigated the roof and building envelope for potential water intrusion pathways. The end cap on the ridge flashing was found to be open and had been damaged by birds.
Based on these findings, water intrusion through the bird-damaged end cap on the ridge flashing was determined to be the likely source of the observed mold growth.
Building Maintenance (Roofing) removed and replaced damaged end cap on the ridge flashing.
Building Maintenance (Carpentry) repaired the damaged drywall ceiling.
