Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

The Office of the Environment assessed the closet in the art room in response to a staff concern.
No visible mold growth was observed within the closet or above the ceiling tile.
No evidence of water intrusion was identified within the closet or in the area above the ceiling tile.
An exhaust fan was present within the closet and was operating properly at the time of the assessment.
Significant dust accumulation was observed on the floor, shelving, and stored items.
The corrective actions listed below were implemented to reduce dust accumulation, which is the most likely source of the reported concern.
The Office of the Environment informed school staff and custodial staff of visible dust deposits and recommends staff working together to develop a cleaning plan for the area.
The Office of the Environment recommended that staff limit paper items as much as possible or storing items in plastic bins with lids to limit dust.
