Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)
Date of Assessment: June 17, 2026
Room D6
The room consisted of concrete masonry unit (CMU) walls, vinyl floor tile, and dropped ceiling tile.
No visible indications of leaks were observed within the space except for two stained ceiling tiles.
Walls and ceiling tiles were assessed using an infrared camera; no thermal anomalies indicative of active water intrusion were identified.
No visible mold growth was observed below or above the ceiling tiles.
Visible dust accumulation was noted on shelving, counters, the paper towel dispenser, and other surfaces.
Science Storage Area
The room consisted of concrete masonry unit (CMU) walls, vinyl floor tile, and dropped ceiling tile.
No visible indications of leaks were observed within the space.
Walls and ceiling tiles were assessed using an infrared camera; no thermal anomalies indicative of active water intrusion were identified.
No visible mold was observed above or below the ceiling tiles.
Visible dust accumulation was noted on shelving and other surfaces.
Science Prep Area
The room consisted of concrete masonry unit (CMU) walls, vinyl floor tile, and dropped ceiling tile.
No visible indications of leaks were observed within the space.
Walls and ceiling tiles were assessed using an infrared camera; no thermal anomalies indicative of active water intrusion were identified.
Less than one square foot of visible mold was observed on the door and side of the refrigerator. The source appears to be an incidental spill as food residue was observed in the vicinity of the mold growth. Furthermore, remnants of food was observed on the stove top within the prep area.
The Office of the Environment noted rodent feces within the space.
Custodial staff was informed of visible dust and will work with staff members to develop a cleaning plan for the area.
The observed stained ceiling tiles are to be replaced with new ceiling tiles.
The Office of the Environment used a disinfectant to clean visible surface mold and residue from the refrigerator.
Integrated Pest Management (IPM) to perform an assessment where evidence of rodents was observed.
