Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

Approximately one square foot of visible mold growth was observed on the drywall wall adjacent to a storage hutch.
The remainder of the office was inspected for visible mold growth, and no additional areas of concern were identified.
The walls and ceiling were evaluated using an infrared thermal imaging camera to identify thermal anomalies indicative of water intrusion. No thermal anomalies were observed.
Drywall surfaces were also assessed with a moisture meter for elevated moisture content; no elevated moisture levels were detected.
Based on the limited extent of the mold growth and the absence of evidence indicating an active water intrusion source, the observed mold growth appears to be the result of an isolated incidental spill.
The Office of the Environment used a disinfectant to clean visible surface mold from the drywall. The adhesive securing the vinyl cove base was observed to be failing, which is indicative of moisture intrusion. As a precautionary measure, the cove base was removed to assess whether mold growth was present behind it.
No visible mold growth was observed behind the cove base. However, out of an abundance of caution, the area was cleaned and disinfected.
Building Maintenance (Carpentry) will replace removed vinyl cove base.
