Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

No odors were detected.
The carpet was probed with a moisture meter. No elevated moisture was detected.
The closet contains drywall. The drywall was probed with a moisture meter (drywall setting). No elevated moisture was detected.
The cove base (adhered to drywall) within the closet was peeled back. No visible mold growth was observed.
The boxes that had become wet were moved to the stage prior to the Office of the Environment arriving onsite.
The boxes were assessed. The boxes were dry and water staining was observed. The staining was on the portions of the boxes that were in contact with the carpeted floor. No mold growth was observed on the boxes.
The carpet in the TV Studio was recently cleaned using wet methods. Custodial Services indicated they did not remove the contents of the closet because the cleaning equipment would not fit into the closet. Moisture from nearby carpet cleaning likely migrated via diffusion and was then wicked up into the porous cardboard.
Open penetrations (former electrical conduit) were observed in the floor of the closet. The crawlspace was recently inspected. In general, penetrations are sealed from the crawl space side. As a precaution, the penetrations will be sealed from the top as well.
Areas above the drop-ceiling were assessed. No concerns were identified. No apparent leaks were observed.
Custodial Services was instructed to discard the boxes that had become wet.
Building Services (Carpentry) sealed the open penetrations in the conduit as a precaution.
