IEQ Investigation Process

Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:

  • interview/questionnaire of concern individual(s)
  • inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
  • inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
  • inspection of exterior
  • inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)
Findings

Date of Assessment: September 13 and 21, 2021

The Office of the Environment had visited the site on September 13, 2021. Mold growth was discovered under the drier/washing machine (within the classroom's storage closet). The cause was associated with the drier vent connection (exhausts warm moist air).

The remainder of the room was assessed on September 21, 2021:

No relative humidity mold growth was observed on surfaces within the classroom or storage closet.

A missing ceiling tile was observed near the main entrance of the classroom.

The temperature was 68.4 degrees F, however this measurement was collected early, prior to occupancy. 

Relative humidity was acceptable (59.0%).

A significant portion of the cabinets/drawers were opened to assess for evidence of pests, water damage, etc. No evidence of pests were observed, however several cabinets (green and tan) showed evidence of moisture below the sinks. 

Moderately to elevated moisture readings were collected in the yellow, red, and green cabinets (under the sinks). A stained ceiling tile was located in the storage closet.

An access panel was opened in the center island of the classroom. Evidence of rodents (fecal matter) was observed within the baseboard cavity, however there does not appear to be a significant exposure pathway. 

Dust was observed in cabinets.

Corrective Actions
September 13th, 2021

Custodial Services cleaned and disinfected the mold growth associated with the drier vent.

September 29th, 2021

The contractor servicing HCPSS drier vents was utilized to evaluate and correct the connection.  

September 29th, 2021

Building Services sealed voids (as a pest management practice) on the top of the floor board of each cabinet beneath a sink.

October 6th, 2021

Building Maintenance to place snap traps (no bait) as a screening method for additional pests.

November 24th, 2021

A restoration company was utilized to clean the inside and tops of the cabinets.
