Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

No malodors were immediately sensed upon entering the room.
A faint odor ("plastic") was noticed in the vicinity of the rooms chair mat. This odor was also noticed in the carpet upon removal of the chair mat.
A faint "new" odor was observed from behind the miniature refrigerator stored within the office.
Onsite staff reported the odor had been emanating from the closet and was believed to be associated with pests.
Areas above the drop ceiling were assessed, with no malodors identifed above the drop ceiling.
The room has baseboard heat running along the wall. A metal frame is located around the coils. Mouse nesting material and mouse fecal matter was observed within the metal frame, within the closet. The cause of the odor was likely this material. The proximity to the rooms heat source likely exacerbated the odor.
It is recommended the refrigerator be removed to improve energy conservation.
Consider removing the chair mat. The Office of the Environment has responded to multiple concerns associated with chair mats trapping moisture, eventually causing mold growth/odors within the carpet.
The Office of the Environment recommended custodial staff work with Building Maintenance (HVAC) to have the metal framing around the baseboard heater removed. Upon removal Custodial could HEPA vacuum and disinfect the impacted area.
