Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:
- interview/questionnaire of concern individual(s)
- inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
- inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
- inspection of exterior
- inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)

January 24, 2019
The weather was rainy.
A slight odor resembling dirty water was identified in Room 156 by the Office of the Environment. A source was not identified.
An odor was not sensed by the Office of the Environment in the adjoining rooms (155 and 154).
No thermal anomalies indicative of moisture intrusion were identified within the Rooms.
No odor was sensed within the Air Handling Unit (AHU) serving the area.
There is a sink located in the main hallway of the pod. Building Services (Plumbing) assessed the trap and did not identify any concerns.
The occasional flooding in Room 156 is to be addressed by Building Services as noted in IEQ report CXCLES201800002A. As an odor was detected by the Office of the Environment in 156, the odor sensed in adjoining rooms by others may be associated with the cause of the occasional flooding.

The weather was rainy, however no odors were sensed in the pod.

Weather conditions were rainy. The Office of the Environment visited the site to check for an odor. No odors were detected.

Weather conditions were rainy. The Office of the Environment visited the site to check for an odor. No odors were detected.
If the odor returns, Administration is to inform the Office of the Environment immediately. If available an individual from the Office of the Environment will visit the site to attempt to identify a source.
