Concern
The past few days when picking up and dropping my children off in the cafeteria for extended care, there has been an odd mildew/moldy odor by the side door. It's very strong once you enter the door.
IEQ Investigation Process

Identify deficiencies that may impact IEQ and/or sources of odor concerns. Typically includes the following depending on the nature of concern, but not limited to:

  • interview/questionnaire of concern individual(s)
  • inspection above drop ceiling (condition of roof deck, pipe insulation, return air plenum)
  • inspection of ventilation system (operation of variable air volume box and outdoor air dampers, check controls, measurements of carbon dioxide, temperature and relative humidity, sources near outdoor air intake, measure return and supply air volume, cleanliness of coils, liner and condensate pan)
  • inspection of exterior
  • inspection below drop ceiling (housekeeping, sink and floor drain traps, signs of past and present moisture concern via visual and/or moisture meter, mold growth, ensure connection of current and capping of abandoned sanitary vents, odorizers, excessive plants and fabric items, identify potential pathways, and measure volatile organic compounds, carbon monoxide, and lighting)
Findings

No odors or mold growth were observed at the door way.

A refrigerator is located near the door. No odors or spoiled food were associated with the refrigerator.

The backsides of posters in the cafeteria were inspected. No mold growth was observed.

The condition of the window caulk was good.

The Heating Ventilation and Air Conditioning (HVAC) system that services the area was inspected. No concerns were identified.

An access panel near the door way was accessed. No concerns were identified.

A member of the custodial staff was interviewed. During the interview, it became apparent that the floor (floor tile) was recently cleaned. The equipment used to perform the cleaning was inspected. An moldy/must odor was noted in the water container, indciating the water had not been changed recently. Discussions with the Manager of Custodial Services indicated that the water is to be changed daily.

The odor noted was likely due to not changing the water according to HCPSS standard protocol. 

Corrective Actions

Administration is to ensure that Custodial Services changes water within the floor scrubber daily.
